Clinics are BY APPOINTMENT only. After you submit the form, you should receive an automatic confirmation to the email address that you entered.

If you believe you had submitted the form but did not receive a confirmation,
it is possible there is a glitch in the system, or the email address was entered incorrectly (it has happened more than a handful of times). If you DID NOT receive a confirmation email, please email me at corepawtential@gmail.com to make sure your booking went through.
